How do I make an appointment?

Please call our office at 925-460-8167 to request an appointment or you use our Web Portal.

What are your office hours and telephone number?

Our office hours for the Pleasanton office are: 8:30 am to 5:00 pm Monday through Friday. We are closed from 12 Noon to 1:00 pm for lunch.

Do I need a referral to be seen?

This depends on the type of insurance you have. If you have a PPO plan, then you do not need a referral to be seen at our office. If your insurance is an HMO, you would need a referral from your physician as well as an authorization from your insurance in order to be seen at our offices.

Am I able to fill out my paperwork in advance?

Yes, please download forms to fill out prior to your appointment.

What is your cancellation policy?

We request 24 hours notice of cancellation of an office appointment and 48 hours for cancellation of a procedure. A cancellation fee is applied if you do not notify our office in the time requested or you fail to keep your scheduled appointment/procedure.

How can I request my medical records?

In order for our office to release your medical records, we need to obtain a signed consent form, giving your permission to release the records either to yourself or to another physician. Please go to forms and download the Medical Release form, complete and fax or mail to our office.

Will my procedure take place during my first visit at your office?

No, your first visit will be a consultation with your physician. Your physician will then decide whether or not a procedure should be performed.

May I see a physician and have a procedure if I do not have medical insurance?

Yes, speak with the billing department for special arrangements prior to your appointment.

What should I do if I need a prescription refill and I am not scheduled for an upcoming appointment?

If you need a refill for your medications, please contact your pharmacy first. Allow our office 48 hours to review medication requests. Refills will not be refilled on weekends, after hours or on holidays.

Who do I speak to regarding billing/financial questions?

Call our office, 925-460-8167 and press Option 5 to speak with our billing department.

I have seen a physician at your office in the past, so why is an office visit needed before I can schedule my procedure?

It is important for your doctor to update your medical history and assess you prior to scheduling a procedure in order to ensure your safety. Also, many insurance companies require a current documented office visit prior to authorizing a procedure.

Do I need to bring anything with me for my office appointment?

Yes. Bring your medication, insurance card, picture ID (Driver License). Also, a referral if required from your primary care physician, and additional test results or pertinent medical records. We suggest you fill out the medical forms ahead of time in order to expedite your office visit. Please see our New Patient Forms section.
